This is the IdiotBox Effects Super Bass-o-Matic '76 envelope filter pedal. Here's a rundown of the controls:

-SENS: This sets the threshold level for the filtering effect. Turn it clockwise to trigger earlier. Turn it counter-clockwise to trigger later.
-BLEND: This is a blend between the dry signal and the wet/effected signal.
-FREQ: This adjusts the width of the frequency band of the filtering.
